    No online source to recommend, but there's a good account of the evolution
    of the 12" single in the book "Last Night a DJ Saved My Life" by Bill
    Brewster and Frank Broughton. Apparently, the first 12" single ever pressed
    was a special Tom Moulton mix of "So Much For Love" by Moment of Truth. It
    was the first of several remixes Moulton pressed up for his personal DJ use.
    The first record-company sanctioned 12", for promotional use only, was
    "Dance Dance Dance" by Calhoun, in 1975, and the first
    commercially-available 12" was "Ten Percent" by Double Exposure, in 1976.
    (All disco tracks.)

    "Last Night a DJ Saved My Life" has much, much more to recommend it. Quite
    simply, it's an essential read.
